So you have some new or gently used equipment you want to donate to L.A.C.E.S.? Great! Here’s how to get started...

| 1. Print out THREE copies of the attached label. |
2. List the items you’re donating in the “General Description of Contents” field. |
| 3. Add up the value of your items using the guide below and fill in the “Value in US Dollars” field. | |

4. Measure your package and fill in the “Size of Container” field. |
| 5. Place one label inside your package, tape one securely to the outside of your package, and keep one for your own records | |
| 6. If you have more than one package, please repeat steps 1 through 5 for each. |
7. Ship your package(s) using the following delivery address: |

8. Email Seren Frost at seren.frost@lacesport.org to inform her of your donation. Please include a list of the items donated, the date of shipment, and any photos you have of your team using the equipment that you are donating. |
